is a Medicare Shared Savings Program Accountable Care Organization (ACO) and Value Care Organization (VCO).
APN's for-benefit cause is to reduce costs and improve quality across the continuum of care while allowing disparate healthcare organizations to maintain cultural and financial independence.
Alaska Provider Network provides collaborative and accountable care system support to the ACO participant. The model involves health care initiative programs using the team approach with the patient at the center to provide resources for caregivers to offer enhanced and coordinated patient-centered medical home-directed care for patients. Our programs encompass prevention, standardization of evidence-based best practices, community-based care coordination, investments in health information technology, health information exchange, medication reconciliation, and reimbursement incentives for increasing the quality and efficiency of care for patient populations.

Developing our solution over the course of the last eight years through parent affiliate Pinnacle Integrated Medicine in the Pacific Northwest has afforded Alaska Provider Network nearly a decade’s worth of experience learned and adapted to based on the success and failures of past models.
Physician-led networks are performing more efficiently and comprehensively than hospital networks. The data and results are indicative of this positive, patient-first outcome.
One reason for the superior physician-led paradigm is these groups are nimble and can work in myriad value-based systems. Physician-led groups also achieve savings differently than hospitals by taking care of patients upfront. Without question, hospitals are important to this model because they (hospitals, specialty services, and infrastructure) can be integral to the overall healthcare journey. But the hospital system’s value proposition is different as they manage patients once admitted.
Also, there is a burgeoning roadmap to follow that didn’t exist 8+ years ago making participation easy and without the risk of losing revenue or violating rules and policies.
While we recognize fee-for-service isn’t going anywhere soon, especially in Alaska, there remain incentives for providing value that doesn’t involve risk.
